FMEA 是精益六西格玛和质量改进中的常用缩写。  它是“Failure Modes and Effects Analysis”（失效模式和效应分析）的缩写，基本意思是“格外细致地查看事情是如何以及为何出错的。” 

在质量项目上下文中，将使用 FMEA 来系统地执行以下操作： 

- 确定产品或过程可能发生失效的不同方式
- 记下这些失效可能产生的效应 
- 找出这些失效的潜在原因以及与每个失效相关联的风险 
- 列出并排列为了降低失效风险而可以采取的措施 

如何进行 FMEA 

FMEA 过程通常涉及到以下操作： 

1. 确定过程中每个步骤/输入的潜在失效模式。 
2. 确定与这些失效模式相关联的失效效应。 
3. 确定每种失效模式的可能原因。 
4. 列出为防止这些原因发生而采取的任何控制措施。 
5. 对效应的严重性、出现的可能性以及在造成缺陷之前检测到失效模式的几率分配数值。 
6. 将严重性、出现率和检测的值相乘，得到风险优先级编号 (RPN)。 
7. 改进任何具有高 RPN 的项。记录您采取的措施，然后修改 RPN。 
8. 作为动态文档进行维护。

- 如何将纳入原因频率、失效严重性和提前检测能力因素的风险值分配给过程步骤？ 

- 改进项目应该侧重于过程的哪一部分？ 
- 哪些输入对过程至关重要，哪些则不重要？  
- 作为过程控制的一部分，如何记录应对计划？ 

何时可以使用 FMEA 的结果？ 

FMEA 是一个可以在质量改进项目的早期使用的绝佳工具，因为它收集和组织与过程有关的数据，可以帮助您识别和确定过程改进项目的范围。FMEA 还有助于在项目完成时识别和筛选潜在的关键 X。  在项目结束时，您可以使用 FMEA 来记录状态、参考应对计划并记录随着时间的推移可能需要对过程进行的任何进一步改进。
